This post is embedded within the well established Reablement physiotherapy team at Torfaen Community Resource Team (CRT). The post supports prevention of hospital admissions and early discharge alongside promotion of self-management for patients with long term conditions.

The post holder will demonstrate excellent organisational and communication skills, be a team player, and be able to work as an autonomous practitioner to deliver rehabilitation programmes across Torfaen locality.

You will work as part of the wider CRT and Primary Care & Community MDT to promote and support patients to remain independent within the community. The opportunity to gain wider experience through the Torfaen Falls service is also available within this post.

The CRT provides a hospital to home service and the Physiotherapy role is a vital one in working with individuals to ensure a seamless service in their journey. The successful post holder will be supported by the other Band 6 and Band 7 Physiotherapists within the team.

The post holder will be HCPC registered and able to visit patients throughout the Torfaen locality.

Main duties of the job To be an integral member of the CRT delivering profession specific and generic interventions to people who fall within the Frailty spectrum of care and Community Physiotherapy service.

The post holder will primarily deal with the assessment and treatment of people who require rehabilitation but will not be limited to this and will be expected to support other aspects of the CRT in line with service demands.

  • Diploma/Degree in Physiotherapist
  • State Registered Physiotherapist -HCPC
  • Relevant clinical educators' course or willing to undertake training
  • Appropriate range of postgraduate clinical education
Desirable
  • MCSP
  • Active participation in relevant interest group
Experience Essential
  • Broad range of post graduate clinical experience including comprehensive rotational experience
  • Specific experience in speciality applied for
  • Experience of participating in and delivering in-service training programmes
  • Experience of supervising staff
  • Evidence of having worked unsupervised
Desirable
  • Evidence of having worked in the UK as a HCPC registered Physiotherapist
  • Experience of facilitating learning with undergraduate students
  • Evidence of integrating user participation in service development

  • The ability to discuss the clinical reasoning process in the management of complex cases
