Health and Wellbeing Coach

Part Time (15 hours)

Up to £31,000 pro rata, depending on experience

Durham and Darlington area (mix of remote and face to face)

About the role


We at Connect Health are delighted to be growing our team of talented clinicians and are currently looking for a Health and Wellbeing Coach who is passionate about using their current skills supporting people to reach their own health and wellbeing goals.

Join our motivated, ambitious, and fast-growing team. We focus on competency, developing talent and offer great flexibility.

Here are some of the main duties of the role will see the successful post holder utilising their specialist skills to

  • You will "onboard" patients to our Pain Management Programme this involves calling patients, setting expectations of the programme and booking them into appropriate slots. This equate to approximately 7.5 hours per week.
  • Identify and engage effectively in a nonjudgemental and respectful manner with members of the community from all backgrounds, abilities and ages, listening and interpreting complex health information into understandable terms about health and health improvement.
  • Assess the client's abilities and preferences, thinking laterally and then advising and supporting them in a variety of areas/activities being and sensitive to barriers to adopting a healthier lifestyle such as affordability, accessibility and life circumstances.
  • Encourage people in choices and actions that are acceptable and achievable to them, while being aware of cultural and socialconsiderations in regard people's lifestyles
  • Conduct review of a client's circumstances and challenges to determine the nature and extent of their nonclinical needs.

Who we are looking for:

  • Degree and/or equivalent experience in health care/social care or related area.
  • Hold certificate for (or be willing to work towards) Levels 1 and 2 Safeguarding
  • Adults and Children
  • Experience of working with health sector and multiple stakeholders.
  • Experience of working with patients with challenging longterm conditions
  • Experience of giving advice/education to individuals with long term conditions
  • Experience and demonstrable evidence of incorporating patient/client views into the development of services e.g. coproduction.
  • Good understanding of service improvement, innovation, performance improvement across a range of disciplines.
  • Experience of working with statutory sector.
  • Experience of holistic review of individuals to facilitate the production of an action plan.
  • Experience of working with and supporting volunteers
  • An understanding of the community and social prescribing sector
  • An understanding of community and primary health care sector
  • Ability to use a range of IT programmes
