Respiratory Care Support Worker

The post involves working in collaboration with the Multidisciplinary Team (MDT) to deliver holistic patient centred care.
The post holder will be a member of a core team within a specified area, and will work closely with members of the Multidisciplinary team including Respiratory Consultants, Respiratory Specialist Nurses, ward/service nursing teams, Physiotherapy, and Occupational Therapists, to deliver a high level of respiratory care as prescribed.
You will be expected to complete an in depth, ongoing respiratory focused training programme which will provide the theoretical and practical skills to underpin the scope of practise for this role.
As part of your role you will also be expected to organise community clinic appointments, support clinics with clinical and administrative duties.
As delegated by the registered professional, undertake identified tasks within the framework of a plan of care.
Work without continual direct supervision within an agreed framework, assist patients/clients to achieve physical and emotional well-being and comfort, including assessments of the respiratory condition as required, supporting optimum recovery, within scope of competence.
Ability to identify and support the needs of patients with a range of long term respiratory conditions, including noticing issues and appropriately acting upon signs of acute and chronic deterioration including, but not exclusive to, increased dyspnoea, increased sputum production and/or change in colour, increased cough, and increased use of respiratory medications including inhaler therapy.
Identify and report the social network/support and care infrastructure of clients i.e. carers, family, already known to partner agencies.Ensure that any changes in patient's condition or circumstance are reported immediately to the registered professional as appropriate to the needs of the patient.
To provide administrative duties and support to the Community Respiratory Nursing Team including organisation of home visit appointments, clinic appointments, ordering of specialist consumables, typing letters and reports to other health and social care professionals.

48 general practices (six of which are managed practices), 47 dental practices (including three orthodontic), 99 community pharmacies, 44 general ophthalmic practices (43 providing Eye Health Examination Wales and 34 low vision services) and 17 domiciliary only providers and 11 health centres;
Numerous locations providing mental health and learning disabilities services;
Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.
