Yorkshire Community Healthcare is a specialist Complex Care Provider, who support clients with varying physical, learning, emotional and mental health needs.


This particular role will involve:
- supporting the client at home
- working to personalised support plans
- keeping accurate and up to date daily records
- providing supporting with moving and handling and personal care
- ensuring the safety and wellbeing of the client at all times


For this role we are looking for someone who has previous experience as a support worker and understands the importance of promoting independence and encouraging clients to lead a fulfilling and active life.

Full and part time hours are available.
