We work with adults who have care and support needs, including those approaching adulthood, both in the community and in care placements.

Our approach is rooted in person-centred assessments, with a focus on Strength Based Models of practice and principles outlined in the Care Act 2014.

Key Responsibilities - Conduct person-centred assessments, working closely with individuals and their carers to gather information and positively influence wellbeing.


  • Undertake personalised safeguarding enquiries, demonstrating strong communication and organizational skills.
  • Collaborate with other organizations and professionals, fostering effective multidisciplinary practice.
