Job Title: Care Home In-Reach Team Member

We have an exciting opportunity to join our established Care Home In-Reach Team, offering 30 hours per week. The team accepts referrals for individuals of any age with a dementia diagnosis/memory issues and functional mental health illness over the age of 65. Flexible working patterns will be considered. Based in the peaceful rural location of Belmont Abbey, just 2 miles from Hereford city with free parking available, we pride ourselves on providing outstanding care to patients. You will receive resources for long-term career progression, flexibility, continual professional development, as well as a wide range of wellbeing support. Main Duties of the Job

  • Manage a caseload of patients to ensure timely assessment, diagnosis, and treatment
  • Monitor treatments
  • Effective discharge planning and coordination
  • Undertake physical health observations and necessary actions
  • Monitor, evaluate, and modify treatment plans
  • Apply understanding of dementia effects and provide management advice
  • Conduct complex risk assessments
  • Report incidents and safeguarding concerns promptly
  • Attend multidisciplinary meetings regularly
  • Participate in professional cover/duty
