Excellent Multi-Disciplinary Team including ANPs, Clinical Pharmacists, and Paramedics, Urgent Care Practitioners, First Contact Physio, Mental Health Nurse.

PERSONAL& CLINICAL RESPONSIBILITIES

Provide the full range of NHS Primary Medical Servicesto the patients registered at the practice, temporary residents and patientspresenting for immediately necessary treatment including clinical examination,diagnosis, treatment and referral as appropriate.

Participate in Enhances services delivery.

Be personally accountable for professional and ethicalactions and practices within a legal and ethical framework that adheres to theGMCs requirements and local policies and procedures.

Maintain confidentiality, while communicating patientinformation, in such a way that preserves the dignity and privacy of thepatient and the patients family and carers.

Work in a non-judgemental anti-discriminatory way, withregard to the cultural and religious beliefs of individuals and groups.

Ensure that prior to any course of action involvingindividuals/groups, their informed consent or other valid authority has beenobtained.

Promote and maintain optimum health of the registeredpatient population of the practice by identifying, planning and undertakingspecific health promotional activities with identified individuals and targetgroups in order to improve their health and facilitate further servicedevelopment.

Establish, develop and maintain appropriate systems forthe management of patients with long-term conditions.

Ensure that record-keeping is managed through thecreation and maintenance of concise, contemporaneous computerised records, andthat all care is recorded accurately and in a timely fashion.

Interpret and explain complex facts in relation to apatients condition, taking appropriate action and giving advice in aprofessional manner to team members, and when appropriate to family and othercarers, in such a way that protects and preserves the patients dignity.

Have continuing responsibility for Child Protection andChildren in Need in all cases of suspected or confirmed abuse, and to ensurecompliance with local and national guidelines, policies and procedures.

Ensure that concerns and identified potential risks arereferred to the appropriate agency immediately.

Manage child protection cases efficiently and, whenappropriate, to attend child protection supervision, to act accordingly toprotect children at risk of harm and to have a clear knowledge andunderstanding of child protection issues.

Be responsible for appropriate referrals, ongoingsupport for the patient, family/carer and the co-ordination of other services.

Provide additional services and enhanced services withinthe post holders clinical competence.

Maintain clinical competence and exercise awareness ofprofessional responsibilities by reading current relevant literature and bytaking opportunities for personal and professional development in accordancewith the requirements of the General Medical Council, recommendations of theRoyal College of General Practitioners, National Health Service and HorsefairPractice Group policies.

Use evidence-based practice to develop and maintain highquality clinically effective and cost-effective primary care.

Participate in annual appraisal and, when implemented,fulfil the criteria for revalidation with the General Medical Council.

Maintain full registration with the General MedicalCouncil at the employees own expense.

Assess and diagnose across a wide range of primaryhealth care conditions, order investigations, plan and provide treatment asneeded and discharge patients.

Assess, tolerate and manage risk and deal withuncertainty.

Prescribe in accordance with regulations and with thepost holders training and registration and in accordance with CCG, and localformulary guidance.

Undertake screening procedures.

Provide telephone consultations as required.

Demonstrate critical thinking and diagnostic reasoningskills in clinical decision-making.

Ensure accurate completion of all necessarydocumentation associated with healthcare and practice administration.

Collaborate with other health professionals and agenciesas appropriate, liaising and communicating effectively with medical, voluntaryand statutory agencies as necessary.

Be able to work with people from a wide variety ofcultures and backgrounds, and people with disabilities and learningdifficulties, and to be able to communicate with them (and, when necessary,their carers appropriately to facilitate their care and understanding of theircare within the service.

Manage ones own clinical responsibilities and to act asconsultant to other clinical staff.

Read incoming mail and indicate what data need specificrecording with the

appropriate Read code.

Record in the medical record why investigations arebeing undertaken.

Analyse the results and record in the medical recordwhat actions are required.

Liaise effectively with all other disciplines and toforge effective links with primary,

secondary and tertiary care to ensure effectivecommunication.
