Prevention Coordinator

Salary:
£29,432 per annum, plus payments for 'On Call' Duty

(pro-rata for part-time working hours)


Safe and Sound Homes (SASH) is a registered charity providing accommodation for homeless young people aged 16-25 in the homes of its volunteer hosts.

The charity has operated since 1994 and currently provides emergency 'Nightstop' and longer-term Supported Lodgings accommodation covering York, North Yorkshire and East Riding.

Job Purpose and Summary


To provide crisis and longer-term support for young people aged 16-25 accessing the homelessness prevention service in the East Riding.

The post holder will be responsible for managing referrals, assessing needs and risk and providing specialist one to one support to help young people achieve independence and develop aspirations for their future.


Reporting to:
Project Manager for York and East Yorkshire

Based at:
Home-based (preferably within the proximity of the East Yorkshire area)


Hours: 22.5 hours per week, occasional evening and weekend working required, including delivery of Host Training outside of usual office hours.

Also require participation in an 'out of hours' duty rota (home-based) for which additional payment will be received.

Job

Responsibilities:

  • Manage a caseload of young people facing homelessness including those who are staying in emergency Nightstop accomodation and long-term Supported Lodgings placements with SASH's volunteer Hosts.
  • To carry out initial and ongoing holistic assessments of service users' needs
  • When required, assess the young person's suitability for the Nightstop service and make practical arrangements with the young person and Host for each emergency placement
  • Complete needs and risk assessments for new referrals for the Supported Lodgings service to ensure their suitability for the service and match suitable young people with available Hosts.
  • Complete all referral and move in paperwork, liaising with the young person and Host to ensure all the checks have been completed prior to move in.
  • Provide ongoing one to one support to the young person using a coaching style to support self-esteem growth and manage change and transition to move on positively from the service.
  • Use the Outcome Star to track the young person's progression in service, using this as a focus in SMART support planning to highlight the needs and development of the young person.
  • Support young people to access co-production activities to ensure that they are at the heart of SASH's future development.
  • Support young people to engage in SASH Active activities and to overcome any barriers that may prevent them from taking part.
  • Use a strengths-based and restorative approach to manage issues as they arise in order to help a young person maintain their accomodation, both internally with SASH and externally with other providers
  • Provide appropriate and evidence-based challenge to young people and hosts, including disciplinary actions where necessary.
  • Provide timely and up to date records to the Finance Team to ensure regular payments to Hosts and manage payment of rent to SASH by the young person, following the Rent Arrears Procedure. Maintain accurate financial records including petty cash expenditure.
  • Work in a manner that demonstrates a committment to equality of opportunity and is anti-discriminatory and anti-racist.
  • Promote a multi-agency approach in working with young people, developing positive relationships with a range of agencies including Social Care, Housing, Youth, Health, Education and other charities.
  • Promote SASH services and the referral criteria to local statutory and non-statutory agenices to ensure that the service is reaching the young people who require the service.
  • Maintain accurate and timely case records for all young people, ensuring that reporting to funders is accruate and reflects the true state of the service.
  • Maintain an up to date knowledge of child and adult safeguarding and report any safeguarding concerns to the Designated Safeguarding Lead or deputy as a matter of urgency.
  • Work in a joined up manner that promotes effective communication and streamlined processes across the different SASH teams so that all are able to direct their focus on achieving positive outcomes for young people facing homelessness.
  • Provide 'On Call' duty on a rota basis.
