Opportunities have arisen for Community Mental Health Nurses (Care Co-ordinator) within both East and West CMHTs.

Oldham Community Mental Health Services are currently undergoing a transformation within our service and with the Living Well developments. This means exciting opportunities have arisen for Community Mental Health Nurses within both East and West CMHTs.

The CMHT work with people aged 16 and over with severe and enduring mental health and social care needs. The CMHTs are integrated with the local authority, providing a multidisciplinary approach to the care provided.

Care co-ordinators are responsible for providing holistic assessment, interventions, risk assessments and care plans for their caseload.

We are looking for RMN's, OT's and Social Workers to join our service

Main duties of the job Assessment of health and social care needs

Producing collaborative care plans

Risk assessment and management

Monitoring and evaluating care plans

Liaising with wider MDT

Providing evidence based therapeutic interventions

Providing education advice, emotional and psychological support

To promote choice in service user involvement in care planning and risk management

Ensure practice is in line with social inclusion and recovery models

Identify packages of care to meet the needs of the service user

Ensure carer involvement in the service user's care

To report concerns identified under Safeguarding adult procedures

  • To assess the health and social care needs of individuals referred to the service, holistically, under the agreed procedures.
  • To conduct specialist mental health assessments and Risk Assessments and devise appropriate care plans and risk management plans.
  • To provide evidence based therapeutic interventions appropriate to the service users assessed needs.
  • With regards to Care Act eligibility identify packages of care to meet the needs of those people referred to the team and to implement these packages of care or refer onto other services as appropriate.
  • To perform the duties of a Care Manager in arranging the commissioning of services to ensure the needs of the client referred to the team are met effectively, reviewed regularly and provided according to local authority guidance.
  • To assist service users in applying for individualised budgets under self-directed support.
  • To provide direct assistance, including education and advice, emotional and psychological support to those referred to the team with the aim of promoting re-enablement and recovery.
  • To promote choice and service user involvement in care planning and risk management.
  • To ensure practice is in line with social inclusion and recovery models.
  • To support and work with colleagues, other MDT members and outside agencies collaboratively and in best interests of the service user.
  • To liaise with and provide written and verbal feedback to clients, carers and relevant professionals (e.g. GPs), giving due regard to consent, confidentiality and Information Governance guidelines.
  • To ensure all carers are offered Carer Assessments.
  • To act as an Appropriate Adult under the Police of Criminal Evidence Act 1984.
  • To report and investigate concerns identified under Safeguarding Adults procedures.
  • To complete social circumstances reports for Mental Health Act Review Tribunals and Hospital Managers Hearings.
  • To observe the legal requirements of the current Mental Health Act, Mental Capacity Act, Care Act and other relevant legislative frameworks.
  • To maintain accurate, contemporaneous clinical records on all service user contact, in accordance with current Trust documentation standards, and with due regard to confidentiality and Information Governance guidelines.
  • To complete all necessary inputting, training and documentation related to electronic record systems and the clustering of service users.
Person Specification Education/Qualifications Essential
  • Registered Mental Health Nurse / Occupational Therapist / Social Worker
  • Current registration with appropriate regulatory body (e.g. NMC, AHPC)
  • Completion of appropriate post qualification consolidation e.g. Preceptorship or the assessed and supported year in employment (ASYE)
Desirable
  • Qualification in psychologic therapeutic interventions
  • Mentorship qualification
Experience Essential
  • Substantial experience of relevant post registration practice
  • Experience of managing a caseload
  • Experience of working in a multi-disciplinary team setting
  • Experience of working with people with a range of severe and complex mental health problems
Desirable
  • Experience of clinical supervision of other staff
  • Experience of working within a Community setting
Knowledge Essential
  • Good working knowledge of therapeutic assessment techniques and interventions in the management of behaviour of individuals with complex mental health needs
  • Knowledge of a range of screening and assessment tools and interventions utilised to support individuals with severe and complex mental health problems
  • Comprehensive understanding of Safeguarding adults procedures and practice
  • Comprehensive understanding of the Mental Capacity Act, Mental Health Act and Care Act
  • Comprehensive understanding of roles and responsibilities under the Care Programme Approach
  • Understanding of key national and local policies and drivers in relation to adult mental health service
Desirable
  • Knowledge of local authority and CCG commissioning processes
Skills & Abilities Essential
  • Ability to undertake the care coordinator role . Evidence of ability to provide specialist mental health assessment and risk assessment / management and be accountable for decision making .
