£28,500 per annum

Job Description - What you'll do

  • Part of the Intake team, responsible for assessing and accepting new client referrals for domiciliary and reablement contracts.
  • Coordinate with Area Care Managers to ensure smooth transitions and high-quality care for clients.
  • Build and maintain relationships with external stakeholders to contribute to business growth.
  • Work Monday to Friday from 9 am to 5 pm with rotating On-Call duties.

Qualifications - What you need

You should be an efficient and organised Care Coordinator who is passionate about community care, adept at managing rotas, and skilled in building relationships with care workers.
