We are seeking to recruit a full time Community Enhanced Wound Care Band 6 nurse to support the community team and to run a leg ulcer/Wound clinic both in a clinic setting.

Although your main role will be working within the Integrated Community Team and leading a leg ulcer/Wound Clinic in the locality plus supporting registered and unregistered staff to achieve competence, skill and experience with Tissue Viability whilst improving the care and management of patients, you will also be expected to support the recruitment, induction, clinical supervision and day to day/operational management of the Integrated community Team by taking on some Clinical team leader responsibilities, and ensure an effective contribution to the implementation of SHFT clinical, governance and other strategies.

You will be expected to maintain clinical skills and competence, you will have the ability to work unsupervised, be able to prioritise your workload, have excellent communication skills and the interest and enthusiasm to promote best practice in tissue viability. As a team player, you will be able to prioritise your work in order to meet deadlines and have a patient focused approach.

Access to a vehicle and possession of a full driving license is essential.

Main duties of the job

We advise all EWCNs and Matrons to have a discussion with the team regarding their role, and the referral criteria and process, to ensure clarity. This role can have a huge impact on the support for staff, satisfaction for patients, and reduce the number of visits and length of time some of these wounds are present for.

We are currently working closely with other NHS Trusts to integrate all our collective community, mental health, and learning disability services. Our new organisation, to be known as Hampshire and Isle of Wight NHS Foundation Trust, is set to launch in July 2024.

The transformation will happen in staggered stages; Hampshire CAMHS, part of Sussex Partnership NHS Foundation Trust joined Southern Health in February 2024, with Isle of Wight NHS Trust mental health and community services transitioning in early May 2024. The final combination of Southern Health and Solent Trusts is anticipated to occur in July 2024, when the new organisation will be formed.

Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.
