Strabane Health Improvement Project (
SHIP), Derg Valley Care (DVC) and Ulster University have partnered to deliver the Local Solutions to National Issues project.

This project aims to establish if a social prescribing (SP) service combined with Grow Your Own type initiatives could effectively target both the issue of food poverty and improved mental wellbeing within both urban and rural areas.

FUNCTION


To participate in the Local Solutions to National Issues project by delivering a Social Prescribing Service to clients in Strabane town based on referrals made to
SHIP.

To manage client referrals from participating GPs and other primary care health professionals, providing holistic assessments, co-designing a social prescription to improve health and well-being outcomes for individuals with a longer-term outcome of reducing the number of clinical /medical interventions required.

To work collaboratively with the key partners Derg Valley Care and Ulster University to ensure the project is delivered effectively.


PRINCIPAL RESPONSIBILITIES

  • To undertake holistic assessments and co-design Health and Well-being Plans with individual service users, identifying support needs to ensure maximum engagement in improving health and well-being.
  • To provide service users with continuity and a coordinated experience of care, remaining point of contact throughout the individual's social prescription.
  • To work in partnership with staff from Derg Valley Care and Ulster University to meet the requirements of the Local Solutions to National Issues Project.
  • To work collaboratively with DVC and researchers assigned to the project to deliver the objectives of the project.
  • To ensure information on sources of support is up to date at all times to enable effective and accurate signposting and linking of individuals with services.
  • To raise awareness and develop GPs and primary care health teams' knowledge on how to identify patients suitable for social prescribing service referral on a regular basis.
  • Maintain and update the computerised system with comprehensive data and evaluation systems, including individual health outcomes based on the two assessment tools utilised. (Wellbeing Star & SWEMWBS).
  • Meet on a regular basis with representatives from all stakeholder groups to support the on-going development, monitoring and evaluation of the programme.
  • Provide quarterly comprehensive outcome focused reports detailing the progress of the service, case studies and good practice examples.
  • To keep records of your work and adhere to confidentiality, information sharing protocols and provide monitoring information as required.
  • To ensure you have an understanding (appropriate to your role) of, and comply with
    SHIP's procedures for promoting and safeguarding the welfare of children and vulnerable adults.
  • To implement the principles of

Personnel Specification.

Applicants must have:

  • At least one year's experience of working with resilience with people who have care or support needs. (In a paid or voluntary basis)
  • Excellent communication and interpersonal skills which can be tailored to meet the needs of different audiences.
  • Ability to develop excellent partnership working practices across a wide range of sectors and disciplines.
  • Ability to work independently within a targetdriven and timelimited system, dealing with issues pragmatically and through brief interventions.
  • A commitment to improving peoples wellbeing through practical community based interventions.
  • Excellent ICT skills
Desirable Skills

  • A knowledge of social prescribing and its benefits to recipients, the NHS and the wider community
  • Experience of strengths based assessment, support planning and motivational interviewing techniques.

Job Types:
Part-time, Fixed term contract

£13,702.86 per year
