AIMS OF THE ROLE


The Out and About service is intended to support older people on a short term basis to retain their independence and wellbeing following a personal crisis such as illness, hospitalization, or other issue such as bereavement.

This will usually involve supporting them to regain their confidence and their ability to undertake activities such as taking a walk, meeting friends or visiting shops.


You will work with the older person to identify their goals and then support them to carry out achieve these goals building up towards this over several weeks where necessary.


This is an exciting role for someone who wants to ensure that older people are active in their community, are supported to make social connections and lead healthy, rewarding lives.


We adopt a strengths-based approach in our work, starting out with the belief that all older people have strengths and resources and we can support them to build on these.

You will ensure that services are accessible to everyone, including those who may be hard of hearing, visually impaired, or have reduced mobility.

We will support you to grow as a professional and develop your skills and talents, including supporting you to gain new qualifications where appropriate.

You will have regular informal and formal supervision within this role.

RESPONSIBILITIES AND DUTIES

  • To support older people to identify their own priorities for making social connections and maintaining or increasing their independence
  • To work with older people help them achieve priorities they have for themselves, for example relating to socialising or mobility
  • To undertake a wide range of activities, which increase people's mobility, cognitive abilities and skills
  • To support older people to access activities in their community, including services delivered by Age UK York and external organisations.
  • To resolve problems and concerns raised by older people using to the service, following appropriate policies to manage situations
  • To ensure that the health and wellbeing of older people using the service is promoted, managing all risks and safeguarding concerns speedily and effectively.
  • To work alongside other health and social care agencies to ensure services are coordinated and that older people receive holistic support that meets their interests and needs.
  • To ensure that case notes are recorded accurately on the Charity Log database.
  • To capture data and case studies that will support reporting on the service.
  • To attend and contribute to team meetings.
  • To attend supervision sessions with the service manager.

PERSON SPECIFICATION

Essential criteria

  • A commitment to supporting older people to improve their lives
  • Demonstrate excellent communication skills
  • A team player willing to work alongside other services
  • A creative approach to problem solving
  • Ability to break down a target into smaller achievable elements and work methodically through them
  • A willingness to try new ideas, learning from both failure and success
  • Able to travel between different locations in the city
  • Ability to use office software (Outlook, Word, database systems)


Travel within York will be necessary in this role and you will need access to some form of Transport (car / /bicycle).

Age UK York pays mileage for travel undertaken for business purposes.

Desirable criteria

  • A qualification in health and social care or equivalent qualification
  • Experience of working with older people with a range of needs including frailty, dementia or people with sensory issues.
  • Experience working in the voluntary or charitable sector

Salary:
£21,257.00 per year
