Job summary

We are looking for a community support worker to work in ourHome Treatment Team based at North Devon District Hospital. The successfulcandidate will be working on a shift based rota (including weekends but excluding nights).

Main duties of the job

Our Home Treatment Team (HTT) is a community based servicewho support people experiencing severe mental health issues and who require'crisis' support. The aim of the team is to help people to manage and resolvetheir crisis through assessment and treatment in their home environment. OurHome Treatment Team also supports individuals who are being discharged frompsychiatric hospital, enabling them to continue their recovery at home.

Job description

Job responsibilities

The successful candidate will become part of a multidisciplinary team carrying out Home Treatment Team visits and telephone calls.

Carrying out interventions from collaborative care plans that includes families/carers, and signposting toappropriate agencies. This can include the creation of future support plans, safety plans and completion of physical health observations.

The support worker will provide therapeutic engagement to service users in crisis, Supporting them in a co-constructed manner, to move away from crisis point and focus on desired future goals and solution building.

The post holder will be able to demonstrate excellent communication skills with both service users, carers and organisations. You will be highly motivated and display a good ability to work autonomously where possible. The support worker will work with service users with the full range of mental health diagnosis's who find themselves in crisis.

The support worker will have clear pathways of escalation and will work within the bounds of existing DPT policies and procedures as required.

The post holder is required to hold a UK driving license tocarry out home visits in pool cars.
