At the Community Neuro and Stroke Service, we provide specialized, multidisciplinary care to adults over the age of 18 who are living with neurological conditions, such as stroke, Multiple Sclerosis, Motor Neuron Disease, and Parkinson's Disease.

Your Role:

As an Occupational Therapy Assistant Practitioner, you will be a crucial member of our team, working under the guidance of Occupational Therapists to deliver exceptional care and rehabilitation services.

This is a permanent part time position (30 hours a week).

Main Responsibilities

Your responsibilities will include:

Clinical Excellence:

  • Deliver and support the assessment and management of patients care and rehab needs with indirect supervision.
  • Provide personal care using a rehabilitation approach, fostering independence in patients.
  • Assess for and provide equipment for patients.
  • Support the changing needs of patients with progressive neurological conditions, including towards the end of life.
  • Prioritize and manage your own patient caseload and schedule.
  • Participate in quality audits and risk assessment.
  • Report incidents and seek continuous improvement.

Service Improvement:

  • Contribute to service development initiatives.
  • Adapt your practice in line with team decisions.
  • Help create local guidelines, protocols, and standards.

Health, Safety, and Security:

  • Collaborate on formal risk assessments with the lead clinician.
  • Maintain infection control and safe work environments.
  • Comply with lone working and equipment use policies.

Personal and Professional Development:

  • Take responsibility for your ongoing learning and performance.
  • Participate in supervision and annual appraisal.
  • Share knowledge gained through training, courses, and shadowing.
  • Embrace feedback as a learning opportunity.
  • Share insights and research findings with colleagues.

You will have:

  • Demonstrate an ability and willingness to participate in the competency training of specific knowledge and skills needed for the job.
  • Experience of working within a rehabilitation setting
  • Ability to carry out detailed observations & functional assessments with service users.
  • Ability to work in an interdisciplinary team.
  • Access to a car to enable travel to service users homes including delivery of equipment.
  • Hold an Assistant Practitioner diploma
  • Experience in community services supporting patients in their homes
