An exciting development band 5-6 post has arisen for a Physio, OT or Registered Nurse, to work within the Lymphoedema Team in the West Area of West Sussex.

The post is a permanent contract of 21.5 hours a week based at Shoreham Health Centre.

The successful candidate will have an interest in Lymphoedema and a passion and desire to enhance the quality of lives of patients suffering with chronic oedema. You will be expected to deliver care in various settings including Clinics, Patients own homes and Residential / Care Homes. Access to a car is essential in order to achieve this.

This is a fantastic opportunity for an Allied Health professional or nurse to enhance their knowledge and skills, learning how to expertly assess patients and implement specific care plans. You will receive specialist training from the Lymphoedema Training Academy and be supported by your mentor to complete a Lymphoedema Competency Workbook.

During this induction/training period you will learn the skills required to:-

  • Understand the role and function of the Lymphatic System
  • Expertly assess/diagnose patients with chronic oedema alongside acute/chronic conditions which can be associated with this.
  • Measure for compression garments and wraps
  • Use specialist equipment such as, Doppler, Lymph Scanner, Intermittent Pneumatic Compression Therapy and LED treatment.
  • Collaborative working within primary care as well as in reach to the acute inpatient setting
Main duties of the job

On completion we will support you to deliver education and training to the wider trust.

Our Lymphoedema service is dynamic, forward thinking, and proactive, anticipating the needs of our patients and striving to provide excellent care in the community. The role involves lone working as well as working within the team. We are highly trained specialists with extensive knowledge and skills that are utilized in supporting all patients in a variety of settings. We are passionate about collaborative working and empowering our patients with shared care.

Main Duties:

  • a) Use advanced skills and expert knowledge to assess the physical and psychosocial needs of a defined client group. Implementing therapeutic care plans and treatments based on the best available evidence to improve health outcomes.
  • b) Work within a multi-disciplinary team to ensure effective care plans and treatments are in place for highly complex patients including end of life, palliative care and long-term conditions.
  • c) Communicate highly complex information about patient's conditions to multidisciplinary team colleagues to ensure effective delivery of patient treatments.
  • d) Negotiate responsibilities and support required from patients, carers and other health professionals to ensure correct action is taken to achieve mutually agreed outcomes.
  • e) Referring onto other services or professionals as appropriate.

Job responsibilities

Main responsibilities

Clinical Governance: You will be expected to take part in the processes for monitoring and improving the quality of care provided to patients. This includes risk management, complying with and contributing to CQC requirements and clinical audit.

Ensure that patients receive the information they need and are treated with dignity and respect for their privacy.

The post holder must co-operate with carrying out risk assessments and must adhere to safe systems of work. This includes understanding and adhering to the reporting procedures for clinical and non-clinical incidents/near misses. Employees must attend relevant courses and mandatory updates, take a proactive role in contributing to risk identification and management and be responsive to lessons learned from incidents and near misses.

Reports clinical and non-clinical incidents and proactively manages risk to vulnerable adults.

Timescales for meeting standards of performance relating to the duties and responsibilities identified in this job description will be agreed with the post holder via the annual appraisal process and the relevant KSF outline. To attend Statutory/Mandatory Training.

Teach and educate patients, their carers and other healthcare professionals to recognise subtle changes in their condition that could lead to exacerbation or acute deterioration and take appropriate actions.

Acts as a clinical expert and information resource, leading in training, education and orientation programmes for staff, students and others.

Is responsible for proactively seeking & providing learning opportunities for themselves.

Proactively develops, implements and monitors public health and health promotion activities in accordance with national and local priorities.

Incorporates relevant research findings with practice.

To identify on-going training and updating needs of other health care professionals in the community team.

Service Development

Challenge professional and organisational boundaries. Identify areas for skill/knowledge development. Apply this knowledge to clinical practice providing continuous high quality patient centred health care.

Participate in the development, implementation and audit of protocols, policies and integrated pathways. Use current research that facilitates change in practice which improves clinical outcomes to meet the needs of patients and carers.

Inputs activity onto the recognised Trust database in order to support service development and meet contractual requirements.

Manages risk and maintains service delivery through liaison with colleagues and reporting unresolved clinical/staffing issues to the Team Lead.

Reports clinical and non-clinical incidents and proactively manages risk to vulnerable adults.

Finance

The Post holder will be aware of the trust financial position and the implication to service delivery.

To be aware of the cost of equipment/supplies used and to meet the needs of the patient and service in a cost effective way.

Staff management

Assist as delegated by the Team Leader in the recruitment of staff, appraisal process, risk assessment process and complaints/performance management process.

Assist in clinical supervision and peer review including mentorship to junior colleagues.
