Weare seeking a skilled and compassionate Specialist COPD & Asthma Nurse tojoin our innovative PCN healthcare team.
Asour specialist COPD & Asthma Nurse, you will play a crucial role inproviding specialised care and support to patients with COPD & Asthma. Theideal candidate will have a strong background in COPD & Asthma management,excellent communication skills, and a passion for delivering high-qualitypatient care.
Thefocus of this role will be to specifically focus on improving the quality ofcare to patients with COPD & Asthma across the four MWEB PCN practices thatcover a combined patient list of 37,000 patients.
Thisspecialist role will complement the existing teams of clinicians in eachpractice looking after this specific cohort of patients and will be a variedrole across the four practices and may involve working within aMulti-Disciplinary Team for complex patients.
Main duties of the job

Specific focus will be on diagnosing,initiation and management of medication in patients with COPD and Asthma
Provide Spirometry to enable diagnosisand management of patients where appropriate
Use Feno monitoring where appropriate
Review patients with COPD & Asthmaholistically in order to identify other factors in their lives that affecttheir quality of life that will have an impact on their COPD & Asthma

Reviewing patientswith COPD & Asthma and diagnosing, initiating and managing theirmedications to improve their quality of life and try to achieve their agreed targets.
Work with theexisting practice teams to also ensure their patients with COPD & Asthmahave their other risk factors managed.
There may be times when more complex patients need to be discussedwithin a Muti-Disciplinary Team setting and when this occasionally needs tooccur, you may need to:
a) Consult with patients,their family, and the multi-professional team to undertake assessments ofpatient need and devise and evaluate complex care plans;
b) Evaluate and analyseclinical problems using their clinical knowledge, seeking out and applyingrelevant evidence, enhanced techniques, interventions, and equipment to makeclinical decisions;
c) Delivers enhancedclinical care in the context of continual change, challenging environments,different models of care delivery, innovation and rapidly evolving technologiesusing analysis and their underpinning knowledge to manage complexinterventions;
d) Teach and advisepatients and their families on how to manage their condition
e) Participate in clinicalaudits and research projects where required and implements changes as required,including the development, and updating of practice protocols / guidelines andprocedures locally;
f) Work within NICEguidelines and Northants ICB formulary
g) Recognise boundaries of their practice andknow when and to whom patients should be referred;
h) Demonstrate initiativeand are creative in finding solutions to problems;
i) Hold joint responsibilityfor team performance and service delivery.
As aspecialist nurse there may be a need for key wider responsibilities within thePCN, for example to:
a) Facilitateand support the development and delivery of innovative training and educationto staff, to improve competence and confidence, in line with best practice andguidance related to area of expertise.
b) Participatein and/or lead formal training events promoting best practice in area ofexpertise.
c) Provideprofessional and clinical leadership and mentorship, acting as a clinical rolemodel on the delivery of evidence-based practice.
d) Adviseand support on the development and delivery of work streams relating to Asthma/COPDas part of the PCNs strategy.
e) Toimprove identification, assessment, ongoing support for patients and theirfamily carers.
Person SpecificationExperience
Essential
Person Specification Essential Able to work in different environments, with different Primary Health Care Teams Experienced in the management of Asthma/COPD patients and the initiation of treatment in newly diagnosed Asthma/COPD patients. Degree in nursing Registered with the Nursing & Midwifery Council; Post graduate qualification at level 7 or Masters degree relevant to Asthma/COPD Be on the Association of Respiratory Technology & Physiology (ARTP) national registerQualifications
