Job summary

This post offers an exciting opportunity to join the West Suffolk Virtual Ward which is part of the national Virtual Wards programme, an exciting transformation in how care is planned and delivered in England. The Virtual Ward was established in West Suffolk in late November 2022 and has already cared for almost 1,000 adult patients with a wide range of medical and surgical conditions.

Our Virtual Ward provides specialist led care for a variety of patients including those with respiratory conditions, frailty, AKI, oncology and heart failure, with anticipated expansion into paediatric and surgical patients. This care is provided to them in the comfort of their own homes using an advanced personal remote monitoring platform, enabling them to safely leave hospital sooner (or not come into hospital at all).

In this role you will be working with consultants across a wide range of specialties as well as the Virtual Ward Consultant Clinical Lead and our Advanced Clinical Practitioners. The Virtual Ward team also includes community nurses, specialist nurses and therapists working together to enable patients to avoid hospital admission or go home early.

The team are friendly and dynamic and consistently receive excellent feedback from patients about their experience on the Virtual Ward.

Main duties of the job

General ward work and ongoing care of in-patients including remote ward rounds. Review of patients referred to the virtual ward and face to face reviews of patients as required. Maintenance of ward lists. Handover of patients to on call and hospital at night teams. Attending and supporting consultant ward rounds. Attendance at MDTs. Supporting and working closely with virtual ward ACPs to support the ward round and clinical care for patients.

The West Suffolk Hospital is a District General Hospital built in 1974. It serves a population of around 280,000 in the western part of Suffolk and southern part of West Norfolk.

Medicine

West Suffolk Hospital has a 24 hour Accident and Emergency Department and a full range of Medical, Surgical and other specialties, with a total of around 480 beds. In addition, there is a Day Surgery Unit and a Hospice with a 10 bedded unit. Acute Medicine has 26 assessment beds and short stay ward of 34 beds including an Ambulatory Care Unit. There is a newly developed Cardiac Centre which opened in November 2018 with a wide range of facilities.

Virtual Ward

Delivery and development of Virtual Wards across all systems in England is a key part of national strategy as detailed in 2023/24 Priorities & Operational Planning Guidance paper published in January 2023.

The Virtual Ward in West Suffolk was launched on 28 November 2022 and since that date has accepted patients onto five clinical pathways: (i) respiratory including COPD (ii) frailty (iii) heart failure (iv) intravenous antibiotics and (v) acute kidney injury.

A core clinical and operational team has been established along with support from specialist pathway teams.

Capacity has increased to 40-45 beds and new pathways include general medicine and cellulitis.

Focus to date has been on identifying patients from the acute hospital (early supported discharge) with the ambition to extend access to step-ups from community and primary care teams during 2024.

Patients are based in their own homes with vital signs monitored via a wearable device and remote monitoring platform. Patient kit (including tablet and connection hub) enables monitoring and data transmission, video calls with the clinical team, task allocation, messaging/chat, and feedback via exit questionnaire.

Plans to scale up the Virtual Ward during 2024/25 are under development via expansion of current pathways and implementation of pathways in a number of new specialties.
