An exciting opportunity has arisen for a permanent post as a Band 4 Patient Pathway Coordinator working within the Diabetes & Endocrinology Department within the Medical Specialities Directorate.


The Patient Pathway Coordinator will lead on validating pathways and ensuring patients are not lost to follow-up within the community and bariatric service in Diabetes & Endocrinology.

By working closely with a wide range of clinical and managerial staff and providing high standard of administrative support for MDTs, the post holder will ensure patients receive treatment as soon as possible and ensure national targets are met.


Monitor and review patient care and/or treatment progress using the Patient Tracking List in line with Trust and local policies.


Identify from the Tracking List the next steps in the patient care pathway in line with clinical need (peer review).


Work as a providing a comprehensive administration support to a number of consultants, clinicians and specialist nurses in respect to their general medical duties and specialist work.


Day to day communications with patients, clinical teams which may include those of a distressing or emotional nature, as well as discharge summaries or other relevant correspondence and documents are required.

Working to support MDTs within the service and booking appointments appropriately.

Monitoring the clinic capacity and new and follow up bookings for various clinics

Working off site in Lambeth and Southwark diabetes community clinics and supporting the teams

The Patient Pathway Coordinator will be an exciting and varied role, working closely with the Assistant Service Manager and booking team to support with management of patient pathways, whilst taking on duties to support clinical teams with the administration of work queues, orders and documentation.

The successful applicant will have the opportunity to work closely with administrative and clinical colleagues for a smooth delivery of patient care for the Diabetes and Endocrinology service


Strong communication skills and the ability to work with others within different roles within the Trust are key to this position, as is strong administrative experience and the drive to improve patient care.
