We have an exciting opportunityin our existing City Community Team to employ a Health Care Support Worker. OurHCSW are expected to work seamlessly within the multi-professional communityteam within the South locality and to forge close links GPs and other localteams. We are an integrated co-located multi-disciplinary team which creates awrapped around approach in the delivery of excellent patient care to ourdiverse population. We have good working relationships with our GP colleaguesand secondary care. Within our team we have nurses, care coordinators,community therapists, specialist neurological therapists and Home FirstRehabilitation Support Workers.

Main duties of the job

The community team can treat anypatient over the age of 18 that is most appropriately seen in the homeenvironment. The role of a community HCSW is to visit patients at home toprovide nursing interventions under the direction of registered nursingcolleagues. The focus of the role is to support admission avoidance, to promoteindependence and to assist in the effective management of patients with longterm conditions.

Good general education. NVQ Level 3 in health care or willingness to achieve. Food Hygiene certificate (or willingness to undertake).
