The Health Improvement Lead will:
Provide support, training and supervision to the health and wellbeing coaches to enable them to confidently deliver specialist tobacco harm reduction interventions including smoking cessation support to patients, staff and in some instances advice to relatives/carers of patients Support the Trusts smoke free steering group Contribute to the development and delivery of the Trusts smoke free action plan Proactively support culture change and the promotion of a smoke free environment Provide specialist advice to the Trusts tobacco dependency service Executive Sponsor, Clinical Lead, and champions Provide tobacco dependency intervention and advice to patients and staff with some signposting to relatives/carers of the patients Liaise with pharmacy to ensure that appropriate pharmacotherapy and e-cigarettes are available within the Trust Contribute to the work with the Trusts Communications team to develop and promote internal and external communications around the programme Ensure appropriate data collection to support clinical care and monitoring of the service Contribute to the evaluation and ensure continuous quality development of the programme Please read the attached Job description and person specification for more details about this role
