To be responsible for the accurate and timely processing of prescription requests for the entitled population, liaising effectively with all relevant personnel to ensure prescribing safety for patients and prescribers.

Duties can include but are not limited to, the processing of repeat prescriptions in electronic and hard copy format, including online requests. In addition, the post-holder will be required to support the multidisciplinary team, ensuring the strategic objectives of the practice are met.

This role leads to an overlap of duties within an extensive multi-disciplinary environment internal and external to The Cambridge Practice which requires an ability to work with complexity across a matrix of relationships extending beyond the immediate role and boundaries.

Main duties of the job

Include (but not limited to);

Support the practice clinical pharmacist as required Partake in audit as directed by the audit lead or medicine management team Support the medicine management team in reviewing and updating the practice prescribing policy Complete opening and closing procedures in accordance with the duty rota Maintain office clinical storage and manage clinical deliveries for the practice. Unpack orders received and store safely in accordance to health and safety standards.

The Cambridge Practice was formed in 2018 following the merger of Victoria and Southlea Group practices. We support approximately 23000 patients in the Aldershot area. We have a strong clinical team of GPs, PAs, ANPs, Nurse Associates, Clinical Pharmacist, Nursing and Healthcare Assistant staff with additional services provided by our PCN colleagues including Paramedic Practitioners for home visits and First Contact Physiotherapists for self-referrals to MSK assessments.

The Cambridge Practice is an Accredited NHS training practice supporting GP trainees, medical students, student physician associates, student paramedic practitioners, student nurses, trainee nurse associates.

Job responsibilities

Key Roles & Responsibilities

The following are the core responsibilities of the prescription clerk. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

Process repeat prescription requests received by phone, email, fax, post, slips and the online access system, in accordance with practice protocol Produce prescriptions for the GP to sign within 48 hours unless there are queries, in which case Prescription queries will be highlighted to the appropriate member of the Pharmacy Team, or the duty GP Ensure repeat medication is issued in line with practice protocols, are processed and are ready for collection within a 72 hour time frame. Follow practice protocol when dealing with medication requests for medication on behalf of the patient for non-repeat items Actively encourage patients to reconcile medications to facilitate monthly collections, promote electronic repeat dispensing and online ordering Discuss with patients their requirements, encouraging patients to order only what they require Act upon compliance issues, liaising with the appropriate clinician Ensure medication reviews are arranged and maintain an accurate recall system Effectively liaise with external services district nurses to ensure medicaments are arranged for housebound patients; and build relationship with community pharmacies Input data into the patients healthcare records as necessary Deal with all prescription related enquiries in a timely manner Record and reported adverse effects accurately and appropriately, informing the medicines management team or duty GP immediately Read code data on EMIS Web Carry out system searches as requested

Demonstrate sensitive communication styles to ensure patients are fully informed and consent to treatment.

Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating, including those receiving bad news

Act as an advocate when representing patients and colleagues

Produce written documents that evidence the contribution of the medication clerks team to the practice priorities

Utilise and demonstrate sensitive communication styles

Anticipate barriers to communication and take action to improve communication

Risk Management

Manage and assess risk within the areas of responsibility, ensuring adequate measures are in place to protect staff and patients.

Report and record risks identified in practice.

Monitor work areas and practices to ensure they are safe and free from hazards and conform to health, safety and security legislation, policies, procedures and guidelines

Apply infection-control measures with in the practice according to local and national guidelines

Participate in the local implementation strategies that are aligned to the values and culture of general practice

Person Specification

Experience

Essential

Experiencing working in a GP Practice, particularly in an administrative role

Qualifications

Essential

Whilst it is preferred and desirable to have GCSE grade A to C in English and Maths, it is not a requirement.

Desirable

Basic IT skills desirable, do provide in house training. Good communication and customer service skills. Polite and confident. Flexible and cooperative. Sensitive and empathetic in distressing situations. Ability to work under pressure. Experience in a GP practice.
