Job Focus Priority focus:

Work with the Practice Cancer Screening Champions and use data to develop a thorough and live picture of the Practice patient cohort.

Support Practices to understand variation in cancer screening data at a Practice level and seek to understand why variations exist.

Using available guidance and resources, develop innovative strategies to:
o increase bowel and cervical screening uptake by 5% amongst those who have previously not attended. o Encourage uptake of breast screening o Identify the hard-to-reach population groups within each Practice.
- agree with each Practice an area of focus within their hard-to-reach groups. o utilise existing research and conduct PCN level work to understand why these groups might not be engaging in screening. o support Practices to develop strategies to increase screening uptake by 5% within this patient cohort. o support Practices to utilise available resources to tailor approaches to target identified groups.

  • Work with the PCN to develop a local PCN action plan that will help achieve the above Training and Development: Identify training and development needs within the PCN; this may include the use of skills audit etc. Identify gaps in training provision and work with line manager, Service Delivery Manager and other Cancer Care Coordinators to source training to cover these gaps. Act as a point of contact for the three national cancer screening programmes, able to share knowledge where appropriate within the PCN.

Community Partnerships:
Develop community partnerships appropriate to Practice patient cohort e.g.

links with BAME community groups. Work with Local Care Partnership to represent the PCN practices and encourage integrated working to achieve patient outcomes.

Raise the profile of role within community and partner organisations Seek/facilitate patient and stakeholder insight to feed into future work planning and delivery Raising Awareness of Screening:

Identify opportunities for engaging with patient populations, including different communities, to raise awareness of screening and signs and symptoms of cancer Work with all relevant partner agencies to undertake targeted awareness raising of screening, and risk factors and signs and symptoms of bowel, breast and cervical cancer Support Practices to utilise different comms approaches and use appropriate materials for the local patient population to raise awareness Where appropriate, signpost practices and patients to support services at a local level Ensure that Practice are engaging with national and local awareness raising initiatives, and appropriately challenge them where necessary Share good practice: Identify areas of good practice at a Practice level and share across the PCN Share good practice at city-wide meetings with other Cancer Care Coordinators Bring good practice back from city wide meetings and disseminate in Practices within the PCN Understand reasons for variation in approaches between Practices Core Responsibilities Communications and Relationship Management Provide support to Practice Managers within the PCN for the delivery of action plans relating to cancer screening awareness and uptake Provide and receive complex information/data Liaise with PCN and Practice Managers to support Population Health Management.

Develop close and collaborative working relationships with individual members of the Practices, including GPs, practice manager, practice nurses, clinical portfolio leads and Cancer Screening Champions Ensure timely reporting on areas of responsibility Work with the Service Delivery Manager of the programme to achieve programme objectives and engage with other Cancer Care Coordinators as part of a peer support network Collate patient and public feedback relating to cancer screening and ensure that it is appropriately incorporated in work streams Communicate with PCN and Practice staff and with patients using both strong written and verbal skills and demonstrating good interpersonal skills Provide relevant and timely advice and guidance on own portfolio of projects/function Model a collaborative and influencing style of working, negotiating with others to achieve the best outcomes.


Knowledge, Training and Experience Experience of Primary Care Services or other organisations supporting a wide range of members of the public.

Knowledge of health care service delivery, public health delivery or community engagement. Knowledge and understanding of the complexities of General Practice provision.

Experience of delivering projects within Primary Care or equivalent Experience of participation in and contribution to relevant internal and external working groups Experience of managing individual projects Analytical and Judgement Skills Ability to analyse complex facts and situations and support the development of a range of options to improve uptake of a health initiative Problem solving skills and ability to respond to sudden unexpected issues / changes.
