Description of Duties / Work to be Performed: Community rehab, long term condition management, complex moving and handling, equipment prescription, posture management, home and environment assessments, vocational and cognitive management, seating, fatigue and anxiety management Specialist

Experience Required:
NHS experience in community OT with older adults.
Car driver to work across North East Hampshire with business insurance.
OT25

Working hours - Full time 5 days per week including occasional weekends
Duties - Extensive leadership experience, extensive community experience.
Recent NHS experience. Complex manual handling, rehab, rapid response, elderly care, neuro, orthopaedics.
Must be a car driver.
Responsibilities

Conduct comprehensive assessments to determine patients' needs, abilities, and goals
Develop and implement individualized treatment plans to address patients' functional limitations
Utilize evidence-based therapeutic techniques to help patients regain and enhance their physical, cognitive, and psychosocial skills
Provide education and support to patients and their families, empowering them to participate actively in the rehabilitation process
Collaborate with healthcare professionals to ensure an integrated approach to patient care
Regularly evaluate patients' progress and adjust treatment plans as necessary
Maintain accurate and timely documentation of patient evaluations, progress, and outcomes
Accomodation available if required.
