Following the successful launch of the Trusts Single Point of Access in April 2014, the service is now looking for a Triage Clinician.

We are looking for experienced Band 6 Clinician, for the Single Point of Access, to offer a comprehensive triage assessment of referrals in one of the Trusts flagship services, which receives and triages the majority of the Trusts referrals for Older People's Services, as it is a core operational service.


You will currently be in a band 6 post (either substantive or seconded) and have an experience of community working in mental health care services.


The role will encompass, being an active member of the team in triaging referrals ensuring complex referrals are managed safely and effectively to the right service at the right time in a timely manner,.

So to do this you will need to be:

IT savvy, a good communicator and have robust clinical, as well being able to work flexibly and dynamically in a service where no two days are the same.


The team works between the hours of 8am to 7.30 pm, via a negotiated rota, dependant on peak times of activity, and there is an expectation that the triage clinician will work across all the hours of clinical activity rather than just core service hours.

The Single Point of Access Nurse supports and implements the delivery of; most appropriate and best possible care which is evidence based, person centred and collaborative, documents clearly, accurately and timely in accordance with trust policy and national guidelines.


The role of the triage nurse is to undertake the effective scrutiny of referrals, identifying the needs and ensuring that the referral is signposted to the appropriate pathway.

To offer a comprehensive triage assessment of referrals received identifying the most appropriate service for that referral. Ensure that complex referrals are managed safely, effectively and in a timely manner.

Ensure effective identification of risk and respond in a timely manner.

Take a proactive role in the effective triage of referrals by:
Obtaining relevant information

Formulating a decision based on risk and other relevant indicators

Signpost effectively

Oversee completion.


Display excellent clinical skills and professional integrity being able to convey the outcomes of triage assessments in an effective, professional and timely manner.

Provide professional support and advice to other agencies.

Responsible for the smooth running of referral processes in line with the Trust Referral Policies.

Ensure appropriate liaison with the clinical team in respect of complex or urgent referrals.


Ensure all reasonable details of risks and needs are obtained from service users/carers; GP's or referrers as appropriate to ensure that effective signposting can be maintained.

Assisting in facilitating robust management of referrals reducing risk for both service users and teams.

Ensuring Referrals are appropriately signposted to the relevant service meeting all time standards identified by Trust Policy.

Ensuring effective communication with referral agencies, service users, carers and clinical teams.


Liaise with Duty Workers/Team Managers in services as appropriate to ensure effective relationships are maintained and to seek further clarification on appropriateness of referral to ensure effective signposting.

Ensure any safeguarding issues identified are highlighted and escalated through the safeguarding process.

To ensure effective liaison with other teams according to clinical risk and presentation.

To participate in the future development of services, responding to results of local audits including CQC and CCG reports.

Maintain a directory of services/resource including local community details for the single point of access
