PCFT Learning Disability Care Hub is committed to supporting nurses to build upon their experience and skills through the offer of ongoing employment. Newly qualified and established nurses will be offered the opportunity to embed and extend existing skills through a board range of clinical opportunities coupled a commitment to their continuing professional development.

The post-holder will work closely with a range of clinical teams with opportunities across the Learning Disability Care Hub.

Main duties of the job

Service/Ward/Department Summary: The available post is based within Tameside Community Learning Disability Team, which incorporates multidisciplinary working with individuals with a mild, moderate or severe learning disability. We deliver high quality health assessments and interventions to people with complex and challenging needs. The team comprise of community nurses, Occupational Therapists, physiotherapists, speech and language therapists, psychologists, and psychiatry input.

Main duties of the job Role Summary: Delivery of evidence based health and behavioural assessments and interventions to adults with learning disabilities who have complex and challenging needs. You will be working in a multi-disciplinary context with the expectation to be person centred, creative and innovative in meeting the health needs of individuals whilst offering support to families and carers. There will be the opportunity to work across a range of settings and with a range of provider services. There are regular opportunities for continuing professional development and peer supervision with other Community Nurses working in the Care Hub. We are eager to support continual professional development and access to additional training opportunities.
