Unplanned Care Telephone Triage Clinician

The postholder will undertake telephone assessments, providing evidence-based advice and suggested self-care treatment options and onward referral to the most appropriate agency/professional, for patients with a wide range of clinical conditions accessing the Integrated Unplanned Care Services within CHCP CIC.
The postholder will provide the following duties:
- Triage and accept referrals direct from all referral sources into the Community Services and sign posting on to other agencies where appropriate.
- Undertake comprehensive risk assessments associated with the care of patients in order to ensure clinicians' and carers' safety.
- Act as a role model for other staff and students demonstrating high standards of practice and professional conduct.
- Support the Senior Management Team in ensuring all obligations of service delivery are met.
- To clearly identify urgent and nonurgent referrals and allocate patients to the relevant pathway of care.
- To have continuing responsibility to prioritise referrals through the integrated single point of access (ISPA) using sound clinical knowledge and risk assessment skills to ensure patients are transferred to the most appropriate service to meet their health care needs.
