Experienceof arranging discharges
Socialworker Qualification, SWE
Experiencein mental health social work, minimum 1 year

Participate in the multi-disciplinary assessmentof individuals with substance misuse and/or mental health problems orvulnerabilities living in the community.
Ensure that a risk assessment is completed forall service users
Adhere to the Trust confidentiality and IGstandards
Beaware of the potential for vulnerability to abuse amongst the people who usethis service, and use of the local policy to inform appropriate personnel inline with safeguarding vulnerable adults and children and SABP Policies
