An exciting opportunity has arisen for an Advanced Practitioner in the Integrated Disability Service. The team provides a case management service to people with complex needs, usually associated with learning disability.

As an Advanced Practitioner you must be a qualified social worker with at least two years of post-qualifying experience in a statutory setting.


You must be able to demonstrate experience of working in complex situations, adult safeguarding and a good understanding of the MCA and making best interest decisions.

The role is a pivotal one in the team in terms of both carrying a small caseload of complex cases but also carrying out some management tasks in the team.

You must be able to support colleagues with their work and deputise for the team manager as required.

This is through formal supervision as well as mentoring others such as students, apprentices and ASYEs and you will be part of the wider management team of the care group.

Experience of mentoring or supervising others is useful but what is equally important is an approach that inspires and supports others.


You will carry out a range of statutory social work functions such as assessments, reviews, complex safeguarding enquiries and tasks associated with the Mental Health Act.

You will support colleagues with chairing meetings, attending court, as well as carrying out formal supervision. Our APs also support our newly qualified staff through their first year of assisted practice.

There is scope for APs to be involved in project work and areas of research that are of interest to them.


Our social work teams are part of CWP NHS Foundation Trust and as such enjoy all the benefits of working for the NHS whilst maintaining close links with the Local Authority.

There is a wide range of training on offer from both organisations and a clear progression framework for our social work staff.

All staff have regular reflective supervision and appraisals, and a training programme developed for their individual needs.

As an AP you will be able to access the NHS Leadership Academy to further your skills and knowledge on a range of management and leaderships areas of practice.

We also have close links with academic organisations in our area and offer opportunities for further academic study and research.


CWP provides health and care services for local people, including mental health, learning disability, community physical health and all-age disability care - including the provision of three GP surgeries.
