About the Role Clacton PCN has a fantastic opportunity for an enhanced practice nurse to join our team on a full-time basis over 5 working days. Clacton Primary Care Network (PCN) comprises of 6 local GP practices: East Lynne Medical Centre (Employer and lead practice) Old Road Surgery North Clacton Medical group Thorpe surgery Fronks Road Harewood surgery The successful candidate may be required to work at any of the PCN member practices dependant on need and demand at time of recruitment. The main responsibilities of the role include: Undertaking health assessments, differentially diagnose, plan, implement and evaluate treatment interventions. Providing and evaluating a range of health interventions to ensure that patients' health and social care needs are met. General health and well-being promotion as well as creating individual, person-centred plans for patients with, or at risk of, a long term condition. Managing and treating long-term conditions, in line with national and local policies and the practice needs. Reviewing medication for therapeutic effectiveness, in accordance with evidence-based practice as well as national and practice protocols; working with patients to support their adherence to prescribed treatments. Prioritising health problems and intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
Please see the attached job description for further details. About the Candidate All applicants should have the following qualifications, registrations, skills and experience: A relevant nursing degree. Current NMC registration with an active PIN.
Has a post graduate qualification at level 7 or above. Completed Consultation & Diagnostics or Holistic Assessment module. Hold an independent prescriber qualification Compassionate manner with an empathetic approach and demonstrable kindness in all aspects of your work. The successful candidate will value the dignity of our patients, their relatives and carers, as well as their colleagues at the practice. Knowledge, and experience of its application, in supporting long-term conditions. Insight and understanding of current issues in nursing and the wider NHS.
About Us

Our primary care network has various clinical and non- clinical roles which consist of the following: paramedics, clinical pharmacists, physiotherapists, cancer care nurse, care coordinators, GP assistants, adult mental health clinicians, physician associates, children's mental health worker and children's councillor as well as social prescribers. All of these roles work towards bridging the gap between primary and secondary care as well as supporting our member practices to improve the assessment and service that we collectively provide.
