We have an amazing opportunity for a Social Worker to join our Integrated Adult Mental Health team at Wokingham Borough Council on a full-time, permanent basis.
As a Social Worker, you will provide specialized social care to individuals with enduring mental health conditions, working closely with health colleagues and focusing on statutory duties under the Care Act, MCA, and MHA.
You will be part of a passionate and dynamic team collaborating with The Health Trust to enhance services and develop a dedicated social care pathway.

Key Responsibilities:
- Manage a mental health caseload with varied complexity, offering care and services to support individual recovery.
- Apply a strengths-based approach, maintaining professional relationships with customers and carers.
- Collaborate with stakeholders to assess needs and identify optimal solutions.
- Effectively prioritize workload, manage time, and escalate issues when necessary.
- Establish relationships with stakeholders for multiagency working.
- Engage in meetings, 1:1 sessions, and contribute to service development.
- Develop and implement care plans.
Candidate Requirements:
- Registered with Social Work England and experienced in health or social care settings.
- Familiar with Adult and Children Safeguarding Processes.
- Strong communication and collaboration skills.
- Well-organized with excellent time management.
- Proficient in IT.
