Job summary

This is an exciting opportunity for a specialist physiotherapist to support our heart failure and cardiac rehabilitation teams as part of a funded project with NHS England. The post holder will develop an individualised exercise programme specifically for patients with LVSD on heart failure and cardiac rehab caseloads. Training will be given to enable the post holder to deliver the REACH-HF programme as well as using your knowledge, skills and evidence-based practice into incorporating a tailored individualised exercise programme. Using advanced assessment and physiotherapy treatments to adapt the programme accordingly will help to ensure service provision for patients with LVSD across Bradford district. Experience of audit is essential to ensure accurate data is collated and uploaded to NACR and presented to NHS England and the Cardiac Network in accordance with local, regional and national guidelines.

Main duties of the job

We have an exciting opportunity for a motivated, forward thinking and dynamic individual to lead an exciting new initiative to develop and implement an individualised community exercise programme for patients with LVSD. This role will allow the right individual to undertake advanced assessment and physiotherapy treatments of patients who present with long term and multiple conditions and adapt the program accordingly. The role will require service review and evaluation in order to demonstrate impact in collaboration with heart failure, cardiac rehabilitation services and NHS England.

We are looking for someone with excellent communication and clinical skills who is experienced in making autonomous decisions and working within a team. You will have excellent leadership skills and be able to demonstrate previous project work. The role will involve managing a community case load . You will work closely with the Heart Failure and Cardiac Rehabilitation Nurse Specialist Team to develop and evaluate the service.

The post holder will identify and manage referrals for exercise and develop an individualised and holistic exercise regime including face to face, telephone, patient self monitoring programme or offer a hybrid combination to benefit patients with LVSD and reduce inequalities.

A secondment may be considered for the right candidate.

A British driving licence is essential, as is access to a car.

Job description

Job responsibilities

To actively participate in the delivery and evaluation of national and local priorities for LVSD exercise provision.

To work with support from the team as an autonomous practitioner, providing clinical expert advice and support to patients/carers and the wider multi-professional teams involved in the management of individuals with LVSD

Act as a source of information and provide advice regarding patients care and management to medical, nursing and therapy colleagues within the healthcare setting.

Responsible for policy development within own specialist area.

Work with the Service Development Manager to ensure planning of the workforce and future cardiovascular exercise provision in a cost-effective way.

Work closely with fellow Heart failure Nurse Specialists within the team, Cardiologists, GPSIs in cardiology and Cardiovascular Nurse Leads to ensure local and national targets for heart failure are met.

Substantial post-graduate experience at Band 6/7 including recent experience in the NHS Experience of providing heart failure / cardiac rehabilitation exercise programme and leading service development Ability to work autonomously and prioritise own workload Able to support learning and education for patients and practitioners Experience and involvement of clinical audit, research evaluation of service delivery

Desirable

Experience of cross boundary MDT working and working in a community setting Experience of writing a business case
