Knutsford Care Community is looking to recruit an enthusiastic and highly motivated qualified Advanced Care Practitioner (ACP).

You will be joining a well established and passionate team of clinical staff who put patients at the heart of what they do.


The ACP will provide clinical leadership for the integrated team in particular the acute home visiting service enabling people to remain in their own homes and prevent avoidable Accident and Emergency attendances to support adults with a health/social crisis at home.

The service also supports the Discharge 2 Assess initiative to provide timely assessment for rapid hospital discharge.


As the ACP you will work alongside the Care Community Coach and Clinical Lead working strategically, contributing to the planning and development of integrated services, acting as a driver for change and ensuring standards of care and best practice in line with up to date evidence base.


If you are an experienced ACP and successfully completed the transition to advanced practice module and either has or are working towards their Masters' award, then this is an opportunity for you.
