The shift pattern is 37 hour Mon-Fri.

An exciting – and rarely available – opportunity has arisen for a Stroke Specialist Nurse, South-Sector based in QEUH.

The Stroke Service is part of the Rehab & Assessment Directorate within the Acute Division of NHSGGC.

Stroke Service provides assessment, diagnosis and treatment for patients who have had a stroke and provide skilled clinical interventions which encompass a broad spectrum of expertise, health education and health promotion in order to rehabilitate or promote or maintain independent living for patients and support for families, carers & other Healthcare professionals.

You will work as part of the multidisciplinary team to influence and improve practice of other professionals who deliver care to this group of patients.
You will provide an appropriate, high quality, culturally sensitive and effective specialist nursing service to stroke patients in secondary and primary care by working in partnership with service users, care providers and other agencies.

You will participate in the discharge planning process and facilitate the seamless discharge from Secondary to Primary care and will identify patients suitable to receive input from the Community Stroke Team.

You will be the nursing clinical expert in 'Stroke' and be a resource to the multidisciplinary teams providing any ongoing care and participate in the development of the case management package of individual patients. He/she will contribute to the assessment, development and implementation of specialist nursing care programs and advice to patients/relatives/carers across the stroke pathway.

The ethic of the stroke team is to ensure equity and quality service to stroke patients and their families. We aim to support innovation and an evidence based approach to support effective delivery of services. Also supporting the team in an environment conducive to learning and development.
The Stroke Service provides assessment, diagnosis and treatment for patients who have had a stroke.

You will be responsible for the review of ward return patients at weekly outpatient clinics in order to:-

Assess that discharge care package is appropriate
Identify and address any problems since discharge Be a single point of contact for patient/families/carers

In clinics you will provide lifestyle education and risk factor analysis to relevant fast track TIA patients referred by medical staff.

As a nurse practitioner you will be accountable for your professional actions as a specialised nurse within stroke services and act independently within the scope of your knowledge and skills and makes decisions guided by broad policies, protocols and guidelines.

You will have good analytical and judgement skills providing expert advice and guidance as required to the stroke service.
You will be responsible for identifying your own professional needs and evidence this through provision of a personal development plan.
