As an Advanced Care Practitioner, you will be considered a key person in day-to-day clinical leadership and expertise within the frailty team. The Advanced Care Practitioner will demonstrate highly developed specialist knowledge and skills in the management and care of patients living with frailty. You will work autonomously within your sphere of competency as a clinician in a diverse multi skilled team which includes Medical & Nursing teams both internally and within our community Trusts, Occupational Therapists, Pharmacy leads and administrative staff. The Advanced Care Practitioner will formulate clinical decisions and treatment plans to manage the patient illness and injury. They will be part of the wider frailty team and will participate in the development of the service by working with our colleagues in the Emergency departments to pull through patients that meet the frailty SDEC criteria and work to ensure these patients are given specialist care so that they can return to their place of residence as soon as possible. They will carry out holistic needs assessments of patients, initiating interventions and referring to members of the multidisciplinary teams as appropriate. They will exercise advanced clinical expertise, levels of judgement for the development and review of care protocols and training for staff to further enhance and develop out frailty service.

Main duties of the job

Managing own caseload, carrying out comprehensive assessments, being professionally accountable and legally responsible. Assess, evaluate and implement plans of care for patients including their clinical treatment plan. Stream patients to expedite discharge to prevent unnecessary admissions and prevent deconditioning. Be a specialist in the field of frailty, and advise/review as required.

Leadership

Health and care professionals working at the level of advanced clinical practice should be able to: Provide professional and clinical advice to colleagues regarding therapeutic interventions, practice and service improvement. Proactively initiate and develop effective working relationships, fostering clarity of roles within teams to encourage productive working. Communication

Health and care professionals working at the level of advanced clinical practice should be able to: Plan and manage complete episodes of care, working in partnership with others, delegating and referring as appropriate to optimise health outcomes and resource use, as well as providing direct support to patients and clients. Demonstrate excellent communication skills, supporting people in making decisions, planning care or seeking to make positive changes using Health education England (HEE) framework to promote person centred approaches in health and care. Maintain effective professional and productive communications with patients, relatives, the multi-disciplinary team, external agencies, demonstrating advanced communication skills. Communicate highly complex or contentious information with sensitivity, tact and diplomacy. Communicate potentially distressing information and diagnosis to patients and relevant others as appropriate. Keep accurate and complete records of all activities and communication consistently with legislation policies and procedures. Undertake investigation and documentation of a range of adverse or untoward incidents. Refer appropriately on to other members of the multi-disciplinary team as required. Be able to identify and take into consideration barriers to understanding and employ appropriate methods to ensure effective communication particularly where barriers are clinical in nature Provide emotional support to patients and carers Be aware of the procedures to follow if concerned regarding the protection of children and vulnerable adults Ensure compliance with policies, procedures and clinical guidelines for self and others

Service Delivery and Improvement

To develop and implement plans, processes and internal policies to ensure that the Trust conforms to the requirements of relevant legislation Responsible for interpreting, proposing and drafting changes to policies, guidelines and service level agreements (SLA's) that impact the service Proposes changes to own function making recommendations for other service delivery Maintain a good knowledge of emerging policies from government departments and external bodies which may impact the service Contribute to strategic and contingency planning for the service which may have an impact on other areas Proposes policy or service changes Participates in reviews of work practices which impact on service delivery to patients/clients and other specialities
