Bristol, North Somerset and South Gloucestershire health care and voluntary sector partners have integrated services to improve stroke care for our patients with the aim of providing seamless, multidisciplinary care to support local people in surviving and thriving after stroke.

This role will be part of the new team at the Weston stroke sub-acute rehabilitation unit (SSARU).

There will be close multi-disciplinary working with other members of the stroke team and one to one therapy patient care.

• Following competency training, undertake delegated tasks to monitor & review patient treatment/therapy plans.
• Help patients meet their nutritional requirements, observing dysphagia guidance & therapy.
• Support patients with swallowing and communication difficulties in achieving their rehabilitation goals.
• To liaise with members of the multidisciplinary team within the SSARU and local services supporting the pathway of care, patient journey to discharge onwards.
