Adult Social Worker

Main Responsibilities
- To be part of the Grove drug service and HAGA alcohol service multi-disciplinary teams, bringing a strengths based, holistic, whole family and community perspective on case management of client who have multiple disadvantages.
- Develop, implement, and review care plans with substance misuse key workers to promote well being and prevent harm or abuse.
- Ensure care planning enables independence and good citizenship.
- Hold a small case load of clients, who need a team around the person approach and navigation into multiple services including adult social care, mental health and housing.
- Advising and assisting clients of the substance misuse services whose behaviour is facilitating Anti-social Behaviour.
- Undertake Care Act assessment and s.42 enquiries through the safeguarding process in accordance with relevant statutory requirements, identifying risks, needs and options, including self-directed support options.
- Undertake Mental Capacity assessments for service users where it is deemed that they may lack the mental capacity to make a specific decision.
- Complete Human Rights Act assessments for those who have no recourse to public funds but may require support under the Care Act.
- To support the development and implementation of training and workshops for teams in both SMS and social care
- When appropriate ensure there is co-production input to service development
- Liaise with colleagues in own and other departments and external agencies in order to gather information relevant to assessment and care planning activities: act as advocate on behalf of clients and carers within the local authority and with health agencies and voluntary organisations.
- Maintain and update case notes and other records, write reports as required: if required,
- May supervise students, trainees, support staff or volunteers.
- Identify service users who are eligible for CHC funding and undertake checklists and work with other professionals to provide a multi-disciplinary assessment within this framework.
- Intervene in emergency situations to protect vulnerable adults and children and initiate the appropriate statutory or other actions required: undertake s.42 enquiries where needed.
- Undertake case management responsibility for a caseload which includes complex cases, including development of appropriate packages or programmes of care, working in partnership with individuals, their families, carers, and with other agencies; undertake ongoing and urgent case reviews, as necessary.
