The Community Mental Health Team is focussed on delivering a recovery service for clients with severe and enduring mental health issues. We are transforming the way we work to provide outstanding care and building truly rewarding careers. The Bordon and Petersfield Community Mental Health Team operates between the two sites, the main team base being Petersfield Hospital with a smaller base at Chase Hospital in Bordon and some working in the Havant Area. We support our staff in developing their clinical skills in attending post graduate training such as physical health and nurse prescribing. This role, in collaboration with referred individuals and where appropriate family or carers, will coordinate and provide recovery focussed assessments, risk formulations, care planning, crisis and contingency planning, evidence based interventions, response, reviews, reports, discharge planning and attendance at appropriate meetings. Role further includes acting as clinical and case management supervisor and mentor.
