We are seeking a colleague to join us as we develop new approaches to identifying and reviewing patient safety incidents, engaging staff and families and developing meaningful and measurable patient safety actions and improvement.


The Patient Safety Team manages all aspects of incident reporting, incident investigation and incident notification and supports the Trusts' ambition to be an outstanding provider of care.

We will do this by placing continuous improvement and working together at the heart of everything we do so that we can consistently deliver high quality care and make changes that matter to people.

The post holder will be a member of the Patient Safety Team, and be responsible for supporting the Clinical Incident Leads and for working closely with clinical colleagues in the management and governance of clinical incidents, investigations, provision of data and support of services in their programmes of work to improve the safety culture of the organisation.

You will work as part of the team to support learning, quality improvement and positively impact the patient safety culture within our organisation.

The post holder will support the introduction of new tools to support the learning from incidents process, including leading SWARM huddles, conducting after action reviews and sharing the findings.


As a member of the Patient Safety Team you will be at the forefront of improving the safety of care for all of our patients and helping the Trust to become an outstanding learning organisation.


Gloucestershire Health & Care Services deliver community health & social care services for adults & older people via a model of locality-based cluster teams.

Each of the clusters consist of Nursing, Therapy and Reablement Teams working closely alongside social work professionals to provide support to people within their own homes and within bed based reablement units.

We work closely with GPs, local communities, acute and community hospital(s) and other partners to enable people to live as independently and for as long as possible within their own home.
