Job Title: Occupational Therapist at Kent Community Health Foundation Trust

In West Kent, Kent Community Health Foundation Trust is collaborating closely with GPs and Primary Care Networks to enhance health and social care services centered around the new Edenbridge Memorial Health Centre. This presents a fantastic chance to assist local residents by establishing a community hub that integrates key services to offer a proactive frailty model. This setup allows for close collaboration with GPs, urgent care, and community and complex care nursing teams. Additionally, it leverages the advantages of a wellbeing day center and a health and wellbeing team working hand in hand to provide comprehensive care and support to individuals and their families and carers.

Main Duties of the Job:

  • Utilize excellent holistic assessment and clinical reasoning skills to develop proactive and personalized care plans.
  • Engage in on-site work at the health center, clinic, day center settings, and community visits to patients' homes.
  • Build strong working relationships within the health center and wider KCHFT services, and demonstrate effective communication and innovative thinking when engaging with patients, staff, and families.
  • Show interest in supporting undergraduate and apprenticeship clinical programs and supervise junior therapy staff as required.
  • As an autonomous practitioner delivering community care, the role necessitates driving and access to a car.
