We are looking for a clinical Quality and Operational Lead to join our Acute Care at Home Team, and manage the Unscheduled Care Coordination Centre (UCCC).


This post will support the fast developing services under the Acute Care at Home umbrella, providing leadership, drive and strategy alongside the overall management team.


Our Acute Care at Home model is a new Service and has an agreed admission criteria and is working in partnership with supporting services to deliver acute care in the patient's own home.


Acute Care at Home service incorporates the Unscheduled Care Coordination Centre (UCCC), Community Rapid Intervention Service (CRIS) and Community Virtual Ward Service (CVWS) all under one umbrella.

These services are responsible for the referral management & coordination of requests, provision & case management of Virtual wards and 2 hour urgent community response within Staffordshire and Stoke on Trent.


The Unscheduled Care Coordination Centre (UCCC) provides real time access to community health services, for patients who are at immediate risk of attending hospital.

Despite the fact that they are not seriously ill or injured.

It provides a County wide, genuine single point of access to unscheduled care, including two hour urgent community response (CRIS).


You will have a clear understanding into acute models of care and the insight into community services to be able to adapt both services into one clear pathway for patients safely managing their care closer to home using both Organisations expertise and workforce.

You will lead all clinical and non-clinical UCCC Team members as identified in both the role and organisational structure. They will be clinically accountable to the Trust for the delivery of this clinical service.


Based in Smithfield1 Building, Hanley as part of an integrated service between UHNM, UHDB, RWT and MPFT the successful applicants will join an integrated, collaboratively focused team.


The Service will operate between the hours of 8am - 10pm, 7 days per week, and 365 days per year.

The service offers extensive support and knowledge to enable patients to be managed away from the acute setting and to implement the right level of care support to achieve this.

We would expect this role to be flexible and work within the UCCC supporting triage, staff development, documentation and data quality and service development.

We are looking for a number of roles to join one of our community based Virtual Ward Teams and help shape the future of patient care.


Our General Acute Medicine and Frailty Virtual Ward is a new and innovative Service, which has an agreed admission criteria and works in partnership with supporting care services to deliver acute care in the patient's own home.

The aim of a virtual ward is to reduce length of acute hospital stay and/or avoid hospital admission; this leads to quicker recovery and reduces disruption to patients', and their carers' lives.

This means we can proactively manage people living with frailty at home, and for those that have a sub-acute episode we can improve a patient's experience and outcomes.

In addition, this means we have more space in our hospitals for acutely unwell patients and reduces the pressure on urgent care.

The virtual ward NHS England » Virtual wards provides a clinical assessment, treatment, monitoring and review to people in their own home who become unwell and would normally be admitted to hospital or are already under the care of one of the following - Royal Stoke University Hospital, County Hospital at Stafford, New Cross Hospital at Wolverhampton, or Queen's Hospital in Burton, and whose needs can be safely managed at home.

The virtual ward will work alongside and communicate with existing providers across our Health and Social Care System.

Our aim is having up to 410 General Acute Medicine and Frailty Virtual Ward beds available across Staffordshire and Stoke-on-Trent by December 2023.
