We are looking to recruit an enthusiastic Band 4 Health Improvement Practitioner (up to 30 hrs per week) to support the health and wellbeing of people within our core community mental health services.
Using a strength-based approach in line with the implementation of the Community Mental Health Framework, the post will have a focus on the delivery of interventions aimed to address the physical health inequalities faced by people with severe and enduring mental health problems.
The post will be based across the Core Community Mental Health Services and will work in partnership with other services/organisations in the Voluntary, Community, Social Enterprise (VCSE) sector.
