Job summary

To participate in the provision ofa Clinical Pharmacy Service to our member GP Practices defined within thePrimary Care Network geographical location.

Applicants should have completed 18-monthCPPE Pathway course and be an independent prescriber with 2 years experiencein primary care.

Main duties of the job

The role will involve workingtowards various quality improvement projects and to achieve PCN network DES.

The role will have a patientfacing element either face to face or telephone - including chronic diseasemanagement, repeat prescription management and structured medication reviews.This will also include the management and review of patients with polypharmacy.

The post holder will work withintheir clinical competencies to provide specialist pharmaceutical support to allour patients and will be working with members of the wider healthcare team toimprove care and support safe and effective prescribing.

The post holder will need to beproactively involved in quality improvement and audit as well as managingaspects of the Quality and Outcomes Framework.

The post holder will act as thefirst point of call for queries from pharmacists within the PCN and provideleadership skills to coach junior members of the team.

Job description

Job responsibilities

Provisionof routine clinical medicines reviews whilst addressing the public health andsocial needs of patient population.

To reviewand reduce were possible inappropriate polypharmacy and wasteful prescribingthrough timely and effective clinical medication reviews.

Reconcilepatients medicines following hospital discharge and work with patients andcommunity pharmacists to ensure patients receive the medicines they need postdischarge.

Contributeto reductions in medicine related hospital admissions and readmissions bysupporting patients to get the best outcomes from their medicines andidentifying and addressing medicines related issues.

Interface withcommunity and hospital pharmacy colleagues and develop referral processesbetween primary care professionals including the promotion of the repeatdispensing service.

Manage therepeat prescribing reauthorisation process by reviewing requests for repeatprescriptions and medicines reaching review dates.

Managepatients and health care professionals medicine queries.

Increasequality and safety of prescribing through mechanisms such as audit and PDSAcycles.

ManagePrimary Care Network practice formularies to improve the quality, safety andcost effectiveness of prescribing.

Implementdrug withdrawals and alerts MHRA aimed at improving medicines safety.

Work withprimary care professionals and patients to implement NICE and otherevidence-based guidelines.

Runclinics where appropriate to meet the needs of the Primary Care Network anticoagulant monitoring clinics, diabetic reviews etc.

Contributeto multi-morbidity reviews and multidisciplinary reviews.

Providemedicines information and training to Primary Care Network healthcareprofessionals and admin staff.

Reviewdaily Pathology results for patients on known medicines.

Act as asource of medicines information for all of the practice team and patients (around doses, side effects, adverse events, possible alternatives aroundout of stocks).

Undertakeminor ailments triage: dealing with minor ailments and triaging patientsappropriately

Leadership:

Qualifications

Essential

Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council Independent prescribing qualification CPPE Pathway

Experience

Experience and an awareness of the breadth of common acute and long-terms conditions that are likely to be seen in general practice 2 years primary care experience
