An exciting opportunity has arisen within the Breast Cancer Specialist Nursing Team. In the role as Clinical Nurse Specialist you will act as an advocate for patient care, providing seamless and timely management of patients who may or may not have a diagnosis of Breast Cancer. This role also includes reconstruction clinics and breast screening assessment clinics. This fast paced and varied role requires an individual who shows a methodical approach to patient care and able to communicate effectively with the team. Working alongside the Band 7 CNS and the wider team you will help to support patients within a cancer pathway. You will provide an individualised plan of care for patients by ensuring effective information gathering is performed, you will communicate effectively the plan of care both within TRFT and also with other Cancer Hospitals involved in the patients care. This is a permanent contract. The ideal candidate will be able to work effectively with both patients and the wider MDT whilst working cohesively with Breast Nursing Team in the discussion of patient care. This unique opportunity requires you to be committed, organised and an excellent communicator with excellent verbal and written communication skills. You must be able to work as part of a team, help to co-ordinate the caseload of patients using a patient centred approach whilst also supporting each other in working within National Guidelines

Main duties of the job The main purpose of the role will be to provide support for the breast screening assessment clinics, nurse led reconstruction clinics, in the ongoing development of the services whilst also providing exemplary care to patients diagnosed with breast cancer on the case load. The ideal candidate will be an experienced nurse working at Band 5/6 with a keen interest in Cancer services. You must be able to show the ability to provide specialist holistic support and treatment whilst co-ordinating services and personalising the cancer pathway for the patients from their diagnosis, through treatment and also guide patients through the breast self-management programme. Be able to provide essential data collection to the Band 7 CNS and be able to present this information as requested to others. Participate in audit, gather information and be able to input this on to various data collection forums. Act as an advocate for patient care, be able to liaise with multiple professionals across both TRFT and other cancer/general Hospitals, formulate treatment plans and communicate these effectively.
