Job summary

We are creating an Enhanced Care and Support team at the RUH. An exciting opportunity has arisen to join the team as an Enhanced Care Specialist Practitioner/Team Lead.

The team will sit within the Vulnerable People Service. We are an ambitious service. The Enhanced Care Specialist Practitioner/Team Lead role has been designed to deliver specialist care and to operationally lead and coordinate the Enhanced Care and Support team, and will take part in a rota providing cover from 8am - 8pm seven days per week.

The post holder will be an experienced Registered Mental Health Nurse (RMN) with a current valid NMC registration.

As the Enhanced Care Specialist Practitioner/Team Lead you will provide support to the Nursing and Medical staff in the provision of professional, clinical advice and leadership, ensuring that high levels of patient enhanced care is maintained in line with professional and statutory guidelines.

The post holder will influence and improve the quality of enhanced care in the acute hospital setting. This may include people with mental health problems, people with eating disorders, people living with dementia, people with a learning disability and autistic people.

This rewarding position will develop and evolve with our collaborative team working approach to ensure we meet the advancing demand of patients with additional needs.

Main duties of the job

Develop the enhanced care processes to standardise and ensure the safe and effective delivery of care to patients with enhanced care needs.

In conjunction with the matrons, take the lead in reviewing those patients who are deemed to require enhanced care and allocating a member of the Enhanced Care and Support team as appropriate. The post holder will ensure that the Enhanced Care and Support team possess the relevant skills required to undertake their role and ensure they receive the appropriate training.

Work alongside the Dementia Nurse Specialists, the Lead Nurse for Mental Health, and the Lead Nurse for Learning Disability and Autism in reviewing patients to ensure their care needs are met across the Trust.

Operational day to day management responsibility at team level using performance management systems to ensure the effective delivery of activity.

Coordinate the daily allocation of Enhanced Care Practitioners and Enhanced Care Support Workers.

Take an active role in the risk assessment and planning of patient care in collaboration with the ward teams and supporting implementation of strategiesto minimise risk.

Assist with the completion of mental health assessments, including assessment of risk and vulnerability.

Support with the provision of bespoke "on-the job" coaching training to all ward teams with regards to individualised and therapeutic interventions for patients who require enhanced care, role modelling to staff as part of their development.
