Job summary

The role involves managing and maintaining a caseload of colorectal cancer patients, undertaking nurse-led clinics: face to face, telephone and follow-up of patients. A responsibility of the role will be to provide specialist information, support, advice and education to the patient and their family (in-patients and out-patients) from diagnosis and throughout the whole cancer pathway on treatment/management of their cancer.

Main duties of the job

The post holder will work collaboratively with a range of health care providers in the local community and across the regional Network. They will utilise understanding of colorectal surgery and oncology treatment for the purpose of education and management to ensure a seamless service for patients, carers and staff. The role involves managing and maintaining a caseload of colorectal cancer patients, undertaking nurse-led clinics: face to face, telephone and follow-up of patients. A responsibility of the role will be to provide specialist information, support, advice and education to the patient and their family (in-patients and out-patients) from diagnosis and throughout the whole cancer pathway on treatment/management of their cancer.

The post holder will work closely with the multidisciplinary team in hospital to ensure a co-ordinated quality service and will be actively involved in education, audit and research. The post holder will prepare, present and lead discussions in multidisciplinary team meetings on their specialist area.

This job offers a great opportunity to be in a leading role delivering high quality care and support to cancer patients and their families and helping to develop our colorectal cancer service. This includes extending and embedding work already undertaken to improve care for colorectal cancer patients as part of our follow-up programme for patients living with and beyond cancer.

Clinical

To provide specialist nursing advice underpinned by theory and experienceto support patients and their families/carers

To act as a specialist practitioner, planning, implementing and evaluating programmes of care in collaboration with senior colleagues achieved through systematic and holistic assessment, personalised care planning, onwards referral and discharge to primary care team

To provide patients and staff with specialist advice to manage complex symptoms

To develop and share expertise within the multidisciplinary team

To develop and utilize the skills and expertise to act as a resource in the care of patients needs, ensuring an optimum level of service to patients and their carers

To work closely with community and network, other hospital and community staff to ensure an effective seamless service

To present in the colorectal weekly Multidisciplinary team meeting based on the Southmead site

To transcribe radiology requests on ICE following successful completion of the IR(M)ER training course

To deploy high level skills in order to provide effective communication systems of support for patients, including breaking bad news and difficult conversations.

To develop and utilize the skills needed to embed patient and public involvement within the sphere of specialist nursing practice

To ensure that all patients and families are aware of all treatment options, including clinical trials

To work as patient advocate, to help negotiate the patient journey and ensure optimum care is provided to each patient

To nurture and maintain links with other health care agencies and organisations providing support and information to patients

To adhere to and promote in others the importance of infection control guidelines and procedures

To act as a positive role model for nurses in the hospital through the demonstration of excellent communication and clinical skills
