Primary Mental Health Care Practitioner - Valens PCN


An exciting opportunity has arisen for an experienced mental health clinician to work as a Primary Care Mental Health Practitioner within Valens Primary Care Network (PCN) a leading, innovative GP practice with over 50,000 patients based in Northumberland.

You will become part of a team; focusing on supporting the needs of our local population living with mental health and wellbeing issues.


Valens is leading on the development of an integrated care model for adult mental health and has established a new and innovative service by working in a collaborative and multiagency way with our local partners to meet the needs of patients with a range of mental health needs.


In this role you will be working within a multi-disciplinary team; alongside other PCN roles including mental health nurse practitioners and occupational therapists, clinical pharmacists and social prescribers, as well as community mental health, social care, pharmacy, hospital and voluntary sector colleagues; focusing on a personalised care approach to achieve the best possible care outcome for patients.

You will become part of our experienced healthcare team, supporting our core GP services by offering assessment, brief interventions and navigation to local services to both working age adults and older patients.

You should have extensive clinical knowledge and experience across a range of mental health settings.
