To embed enhanced transition packages for Children and Young People with Social Emotional Mental Health Needs (SEMH) at different phases of education as the demands and formality of education increases.  The project will embed a pathway of support for the schools to intervene early for children on a My Support Plan with a need of SEMH.  The intervention will dove tail alongside the work of Team Around the School Mental health Support teams in schools and the Early Help support around SEMH. 

Students will be identified at different stages of their education; it is observed that as learning increases in formality there is a potential for increased intervention on SEMH.  Predominant focus will be to ensure the transition into KS3 is supported. Young people will be identified and the staff team will provide bespoke support to the school to ensure they are able to deliver provision to support the young person through their transition. This support will be overseen by an advisory teacher who will be able to identify appropriate provision to meet need, and this will then be modelled and implemented by support workers to support the school in meeting the young person’s needs.

This bespoke modelling of practice will support consistency of practice across settings and increase staff confidence.

Work will develop an ordinarily available SEMH pathway for Children and Young People that supports their engagement and addressing needs to increase confidence in the educational system from parents.
