Location: Community Based in the Merseyside area with some homeworking – this role will require regular travel within the Knowsley Borough and surrounding areas to support our community 
Contract type: Fixed Term 6 Months (with potential to become permanent dependent on funding). 
 
About the role

This role does not include domiciliary/personal care responsibilities.
 
This role sits within our Community Dementia Adviser team directly supporting people living with dementia and their carers across Knowsley. You will be working within a well-established service, providing both one to one and group-based support as we come towards the end of a long-term contract. This position is to cover the remaining period of our contracted service over the next 6 months, with the opportunity to become permanent if we are successful in obtaining further funding when applying for the new contract. 

You will have the rewarding opportunity to provide support and guidance to people with dementia and their carers; helping them to understand the diagnosis, plan for the future, and remain independent and in control of their lives. You will also support people as their condition progresses, to cope with changes that may occur and navigating the health and social care system.

You will:

- Assist service users to access other local services, by signposting and referring.
- Be responsible for managing referrals, assisting clients in assessing their information and support needs, and managing an ongoing caseload of service users in a person-centred manner.
- Be responsible for keeping an accurate record of your support using a computerised recording system. You will also be responsible for supporting our community-based groups.
- Assist with organising and facilitating the safe running of group services aimed to inform, educate, and support people affected by dementia, including workshops.
- Travel to different locations throughout the Knowsley area to deliver 1:1 support and our groups. Telephone support, emails and administration tasks will be done from home.
- Liaise with relevant statutory and voluntary organisations in the area.
- Be accountable for ensuring all aspects of providing high quality services alongside your Knowsley Team.

About you

This is an exciting opportunity to be at the heart of delivering high quality local support to people affected by dementia, alongside internal and external partners and within a busy and well-established service.

- You will have experience of supporting people within an information/advice/guidance role and be able to deliver support and guidance with a non-judgmental approach.
- You will have strong organisational and time management skills to be able to manage a caseload of service users, and balance community and home working effectively.
- You will be able to travel across the Knowsley Borough independently as well as occasional travel to other areas when required.
- You will have a good understanding of dementia and the needs of those living with dementia and their carers. An understanding of the health and social care system as well as community and voluntary sector support services will also be beneficial. This role may also appeal to those who have worked in another advice/guidance role.
