Applicants are invited for a 10 PA Staff Grade post in Palliative Medicine based at Walsall Healthcare NHS Trust to support the 12 bedded inpatient unit and Community Team.

Main duties of the job

This job will be based in Walsall Palliative Care Centre which hosts a 12 bedded specialist palliative care in-patient unit, Specialist Day Hospice and Community Team. A focus of the job will be to support the care provided on the inpatient unit. Job description
Job responsibilities

Provide specialist medical care, advice, and support in order to provide high quality individualized care to patients/clients in line with local and national guidelines. Work together with other members of the medical team and MDT to ensure excellent medical and holistic assessment and care. Participate in weekly Specialist Palliative Care MDT meetings and other MDTs as required. Carry out basic clinical procedures as necessary including phlebotomy, cannulation occasionally catheterisation. Perform / arrange other necessary procedures for patient comfort such as abdominal paracentesis and blood transfusions. The post holder will not be expected to attempt procedures in which they do not feel competent. Maintain accurate and timely patient records. Maintain excellent prescribing in accordance with local policy. Liaise with primary and secondary care services (and other statutory and voluntary agencies) as appropriate to ensure continuity of medical care. Complete holistic assessment records, discharge letters, letters notifying death, TTO prescriptions and anticipatory prescribing in line with local guidance. Confirm and certify of death in line with local protocol, liaising with the Medical Examiner and Coroners Office where required by law and local practice. Complete the first part of the cremation certificate according to regulations and local protocol. Communicate well with family members/carers. Know limitations of own practice and strengths of others practice: Seek support from senior colleagues and those from other professional groups and specialties wisely and appropriately. Work flexibly with patients in the palliative care ward, clinics, Day Hospice, own homes or Hospital as agreed within job plan. Provide medical support to the prioritisation of admissions to in patient unit. Provide supervision and support for more junior team members. To participate in clinical supervision, or equivalent means of psychological support and professional development. Contribution to the out-of-hours cover for Goscote Hospice. Rota to work 1 in 4 Saturdays on the inpatient unit with telephone support from consultant on call. The would be renumerated with a day off in lieu the following week and adjusted salary. Any other reasonable duties as directed and supported by the senior leadership team to meet needs of patients with resources available.

Teaching/Clinical Governance/Other

To assume the role of educator, both formally and informally, to all those involved in the care of the patient to fulfil Walsall Healthcares strong commitment to training both doctors and nurses. Participate in the induction for new members of staff. To take an active role in undergraduate and postgraduate medical and nursing teaching and training. To work with other members of the team to ensure that evidence from research is systematically considered and practices changed as appropriate. To actively contribute to improving quality, reducing inequalities, and upholding Trust values by innovation, audit, QIP, policy development etc. To complete mandatory training. To commit to continuing professional development. To participate in annual appraisal programme and job planning process. To keep appropriate records to comply with General Medical Council requirements for revalidation. To always adhere to the GMC code of professional conduct.

Person Specification
Experience
Essential

Broad range of general Palliative Medicine knowledge and experience at middle grade level or higher in different settings community and inpatient. Extensive knowledge of NHS and social care services to work effectively with people requiring support from multiple agencies or moving between care settings. Extensive understanding of MCA, Best Interests, Safeguarding, and other relevant legal frameworks. Excellent ethnical reasoning & judgement. Outstanding compassionate communication skills in person & in writing. Ability to make good judgments in face of clinical uncertainty & complexity. Ability to work collaboratively with colleagues and within a multi-disciplinary team. Demonstrates understanding of leadership principles, approaches, and techniques. Can appreciate and adapt leadership behaviors to improve engagement and outcomes and how this impact on others. Demonstrate ability to challenge others, escalating concerns when necessary.

Desirable

Completed a minimum of 12 years' medical work or equivalent experience

Qualifications

Full GMC registration Medical Degree, ie MBBS or equivalent Evidence of CMT competences or GP training or equivalent training or completion at start of post.

Commitment to specialty specific CPD via good agreement with relevant Palliative Medicine courses, conferences, or meetings Ability to plan, evaluate and provide teaching training for a wide variety of audiences. Ability to foster safe and supportive working and learning environments. An understanding of the current NHS environment, particularly in relation to initiatives and issues in Palliative Care and clinical governance.

Collaborates with multidisciplinary and interpersonal teams to manage risks and issues across the organisation fostering a culture of learning and accountability. Ability to evaluate own and others clinical practices and acts on findings using tools such as audit and multisource feedback. Experience responding constructively to feedback compliments and complaints including supporting both families & affected staff. Experience of producing, updating, improving SOPS, policies, and guidelines. Ability to support others to grow skills, clinical governances, and QI.
